Ayushman Card (PM-JAY)
The Ayushman card gives free cashless treatment up to ₹5 lakh per family per year at empanelled hospitals under Ayushman Bharat PM-JAY. It is for eligible poor and vulnerable families, and for every senior citizen aged 70 or above.

How to apply
- Check eligibility on beneficiary.nha.gov.in or the Ayushman App using mobile OTP
- Search by state, scheme and Aadhaar, ration card or name
- If your name is listed, complete Aadhaar e-KYC with OTP or face authentication
- Seniors aged 70 plus can enrol directly with Aadhaar e-KYC, no income check
- Wait for approval, then download the card from the same portal or app
- Or visit a nearby CSC or empanelled hospital (Ayushman Mitra) for help
Good to know
- Eligible families are those listed from SECC 2011 data and families added by states, including many NFSA ration card families and ASHA and Anganwadi worker families
- Seniors aged 70 plus get their own ₹5 lakh top up, not shared with younger family members. Those in CGHS, ECHS or CAPF must choose one scheme
- Every State and UT now runs PM-JAY. Odisha joined in January 2025, Delhi in April 2025 and West Bengal in June 2026

How do I check if I am eligible for the Ayushman card?
Log in on beneficiary.nha.gov.in or the Ayushman App with your mobile OTP and search by state and Aadhaar or ration card number.
Can a senior citizen above 70 get an Ayushman card without income limit?
Yes. Everyone aged 70 or above can get the free Ayushman Vay Vandana card with Aadhaar e-KYC, whatever their income.
Is there any charge to make an Ayushman card?
No. Creating the card is free, on the app, at a CSC or at an empanelled hospital.
