Residence and domicile affidavit format
A sworn statement that you live, or have lived, in a state or place. Used with domicile certificate and ration card applications when other residence proof is missing.

Use it when an office asks for an affidavit of residence, for example for a domicile certificate, a ration card, or a school or college admission.

The applicant signs in front of a notary public or oath commissioner.

Common questions
Is an affidavit enough to get a domicile certificate?
No. It is one supporting paper. Most states also ask for proof of how long you have lived there, such as school records, and the revenue office checks the details.
How long must I have lived in the state?
The required period differs by state and purpose. Read the Domicile certificate page on this site for your state and confirm on the state portal.
Do I need stamp paper?
Many offices want the affidavit on stamp paper with a notary stamp. The value differs by state, so ask the office.
