Name change affidavit format
A sworn statement that your old and new names belong to the same person. Used to correct a name in Aadhaar, PAN, passport, voter ID or driving licence after marriage or a spelling mistake.
This is a sample, not an official form. Offices and states differ. Check this text with the issuing office before you sign, and use the office's own form if it gives you one. Govt License does not give legal advice.
What it is for
Use it when an office asks for an affidavit to change or correct your name. Some offices also ask for a newspaper notice or a Gazette notification, so check first.
Who signs
The applicant signs in front of a notary public or oath commissioner. For a minor child, a parent signs.

Common questions
Is an affidavit enough to change my name everywhere?
Not always. Each office has its own list of papers. Some also ask for a newspaper notice or a Gazette notification. Read the page for the document you are updating and ask the office.
Can I change my name after marriage with this?
It is used as a supporting paper along with the marriage certificate. Check the exact rule with the office, as it differs for each document.
Do I need to use stamp paper?
Many offices ask for it. The value differs by state, so ask the office or a notary.
