Affidavit for a lost document (PAN, marksheet, RC and others)

A sworn statement that a document such as a PAN card, marksheet, RC book or licence has been lost, used when you apply for a duplicate.

This is a sample, not an official form. Offices differ. Check this text with the issuing office before you sign, and use the office's own form if it gives you one. Govt License does not give legal advice.

What it is for

Use it when you apply for a duplicate PAN, marksheet, driving licence, RC, share certificate or other document and the office asks for an affidavit or an indemnity about the loss.

Who signs

You sign in front of a notary public or oath commissioner, who attests it.

Common questions

Do I need to file an FIR for a lost document?
Some offices ask for a police complaint or an FIR copy for documents like a driving licence or RC. Others only need an affidavit. Check the list of the issuing office first.
How do I get a duplicate PAN card?
Apply for a reprint or correction on the official PAN service portals and follow the steps in our PAN card guide. Some applications need no affidavit. Check the portal's current rules.
What if I find the original later?
Do not use it once you have taken a duplicate. Return it to the issuing office as promised in the affidavit.
