  2. Enter your mobile number and verify the OTP
  3. Fill in the personal and family details and upload the documents
  4. Save the application number

Apply offline

  1. Download the rural or urban application form from fcs.up.gov.in and submit it at the district supply office

Documents to keep ready

  • Aadhaar of every family member
  • Proof of address, e.g. electricity bill or rent agreement
  • Income certificate or self declaration (for PHH or AAY)
  • Passport size photo of the head of family (usually the eldest woman)
  • Bank passbook
  • Surrender or deletion certificate if you had a card in another state or area
