Self-declaration format for trade licence
A short signed statement by the applicant that the details in the trade licence application are true and the business will follow the rules.
This is a sample, not an official form. Offices and states differ. Check this text with the issuing office before you sign, and use the office's own form if it gives you one. Govt License does not give legal advice.

Use it when the municipal office asks for a self-declaration or undertaking along with the trade licence application.

SELF-DECLARATION

1. The information and documents given in my application for a trade licence for the business of ________ at ________ are true and correct to the best of my knowledge.
2. The premises are used only for the business stated in the application.
3. I will run the business as per the rules and conditions of the ________, and I will not sell or store anything that is banned by law.
4. The business will not cause a public nuisance, such as noise, smoke or blocked roads.
5. No trade licence of mine has been refused or cancelled earlier. (If it was, I have given the details in my application.)
6. I will tell the licensing office about any change in the business, its address or its owner, and I will renew the licence on time.
7. I understand that if any information is found to be false, the licence can be cancelled and action can be taken as per law.

Common questions
Is a self-declaration the same as an affidavit?
No. A self-declaration is signed by you on plain paper. An affidavit is sworn before a notary or oath commissioner, usually on stamp paper. Use the one the licence office asks for.
Does the self-declaration need a notary?
Usually not. If the office asks for it to be notarised, take a printed copy to a notary.
What if my licence was refused earlier?
Do not sign point 5 as written. Change it to state the facts and mention the earlier refusal in your application.
