Affidavit format for trade licence
A sworn statement made before a notary, used by some municipal offices as proof that the premises and details in a trade licence application are true.
This is a sample, not an official form. Offices and states differ. Check this text with the issuing office before you sign, and use the office's own form if it gives you one. Govt License does not give legal advice.

Use it only when the licence office asks for an affidavit. Many cities accept a self-declaration instead.

The applicant signs in front of a notary public or oath commissioner, who adds a stamp and seal.
